Teaching Assistant (Level 4) - with responsibility for SEND transition and looked after children
We are seeking an experienced and suitably qualified professional to join our team and play a key role in supporting some of our most vulnerable learners. Working closely with the SENDCo, you will plan and oversee transition arrangements for pupils with SEND, ensuring they receive the tailored support needed for a successful move into secondary school. You will also work alongside the Designated Teacher for Children Looked After (CLA) and previously looked after children (PLAC).
Key Responsibilities
Work in partnership with the SENDCo to design, coordinate and deliver enhanced transition programmes for SEND pupils.
Liaise closely with primary schools to ensure effective information gathering and personalised support for pupils requiring additional transition arrangements.
Collaborate with the whole-school Transition Lead to contribute to a values driven programme of transition events.
Build strong relationships with parents/carers, teaching and support staff, partner schools, and external agencies.
Provide both one-to-one and small group support to help pupils overcome barriers and achieve their full potential.
Act as a central point of contact for CLA and PLAC pupils, working alongside families and the local authority.
Communicate effectively with parents/carers and partner schools to ensure identification of PLAC pupils.
Lead the Personal Education Plan (PEP) process for CLA pupils to ensure they receive the academic and emotional support needed to thrive.
Work closely with the Designated Teacher for CLA pupils and the Virtual School team to ensure positive outcomes for CLA and PLAC pupils.
