We are seeking an
organised and committed Transition Co‑ordinator to support a smooth and
positive transition from Year 6 into Year 7. Working closely with primary
schools, parents/carers, curriculum teams and the Head of Year 7, the
successful candidate will play a key role in coordinating transition and
induction processes, with a strong focus on inclusion and early intervention.
The role particularly supports students with SEND, those who are disadvantaged,
looked‑after or previously looked‑after, or who have a social worker.
The
post includes primary liaison, organisation of induction events, use of data to
identify and support vulnerable students, and contributing to Year 7 pastoral
leadership, including deputising for the Head of Year 7 in agreed areas. This
role would suit a proactive classroom practitioner with a passion for
inclusion, safeguarding and pastoral leadership.
Responsibilities
Lead and coordinate a smooth, inclusive transition from Year
6 to Year 7, with a focus on early identification and support of vulnerable
students.
Liaise effectively with primary schools,
parents/carers and internal staff to share information and build strong
professional relationships.
Use transition, attainment and pastoral data to
inform planning and targeted intervention.
Organise and support induction, transition and
admissions events, including primary visits and parental engagement activities.
Support and deputise for the Head of Year 7 in agreed pastoral and leadership responsibilities, contributing to Year 7 outcomes and wellbeing.
